Once completed, print and/or email the checklist to help you acquire all needed documentation before going to a Deputy Registrar Agency to obtain your driver license or identification card.
NOTE: Customers are encouraged to use this checklist to prepare in advance of visiting a Deputy Registrar agency. Use of this checklist does not guarantee issuance of a DL-ID. Issuance will be based on the validity and acceptance of actual documents presented at the time of the transaction. All non-renewable, non-transferable credentials will be issued as Standard DL-ID.
If your current full legal name is not the same as your birth name, proof of name changes must be met.
Documents must be presented to connect your birth name to your current full legal name.
Below are a few examples of documents to bring with you to acquire your DL-ID.
*NOTE: If you have had more than one marriage, you may need to bring documentation from each marriage and/or divorce to connect your birth certificate, passport or U.S. Department of Homeland Security documents to your current legal name.
